The preparer needs to complete and sign the section to verify they’ve answered all the questions as honestly and accurately as possible.

Can I skip these questions?

  • You can skip if you’re the student or the parent/guardian. This section is only for preparers to fill out.
  • Preparers may not skip any of these questions.

How to answer these questions

If you prepared the FAFSA® for someone, you’ll need to:

  • Enter your name exactly as it appears on your Social Security card.
  • Enter your Social Security number and Employer Identification Number (EIN).
  • Enter the name of your affiliation or organization.
  • Enter your permanent mailing address.
  • Sign and enter the date you signed.
